Responsibilities
Operationalize the vision of the ARC for Foothill communities
Build relationships with community providers and create new models of care that make access easier for patients and families
Promote growth and development among providers by creating a culture that embraces inclusion and continuous learning and improvement
Educate and assist community members with determining the most appropriate and least restrictive resources for older persons
Qualification
Required
Masters in social work (MSW) or equivalent education in related field such as nursing, psychology, public health or gerontology
Knowledge of health and other issues affecting older adults
Knowledge of resources and social services available to older adults
Direct service provider to individuals and groups, including family caregivers
Experienced supervisor, including to student interns
Experience creating, coordinating and facilitating community partnerships
Experience in providing community group presentations and trainings
Demonstrated skills with networking and fostering collaboration
Experienced in program development, as well as project management
Working knowledge of multiple software applications, including social media
