Health Aging Systems Planner - Planner Principal State

Responsibilities
Lead strategic planning and implement activities to educate providers, improve clinical practice, and engage stakeholders in dementia risk reduction, early detection and diagnosis, disease management, and caregiver support
Contribute to the Aging and Healthy Communities Unit’s Epilepsy programming including working collaboratively with unit staff and external partners to identify gaps through strategic use of data and build coordinated systems that ensure Minnesotans at risk for or living with epilepsy and seizure disorders and their caregivers have equitable access to opportunities and resources to support their well-being and quality of life
Provide strategic planning expertise and consultation to the Aging and Healthy Communities Unit, its partners, and healthy aging stakeholders related to age-friendly health system strategies, clinical-community linkages, and aging related social determinants of health (SDOH)
Qualification
Required
Three (3) years of experience working within public health, chronic disease prevention and management or health promotion. Experience must include the following:
Counseling and/or providing referrals for adults at risk of or receiving care for a chronic disease
Project management experience that includes designing work plans, offering solutions, evaluating results and conveying outcomes to stakeholders
Working with multi-disciplinary teams and individuals from a variety of backgrounds
Knowledge of the impact that policy and systems changes can have on the quality, delivery, and accessibility of clinical services
Ability to edit, organize, and synthesize relevant information from diverse sources to create public health communications, professional education resources, programs, plans and reports
Preferred
Knowledge of Alzheimer's disease and related dementias including risk and protective factors, signs and symptoms of dementia, quality dementia care models, and important role of dementia caregivers
Knowledge of principles and models of Age-Friendly care
Knowledge of quality improvement processes in clinical practice related to chronic disease prevention and management in care delivery systems
Knowledge of organizational structure, methods and procedures of MDH needed to implement complex projects and to interact appropriately with internal partners and contractors
