They are seeking a Registered Nurse (RN) for their team in Missouri to perform clinical evaluations and ensure quality service for patients receiving home care. Responsibilities

Respects client and employee rights and privacy, ensure the security of protected information, practice in an ethical manner and is compliant to the agency’s compliance program and privacy policy

Follows the FreedomCare’s clinical policy and procedures

Regularly reassesses (home visits) the client’s needs to ensure appropriate plan of care is in place. Properly completes PCCP referral to State to request modifications to plan of care

Oversee the completion of caregiver pre-hire and annual training requirements. Approves caregiver’s competency to complete plan of care tasks (home visits)

Initial and on-going instruction of care to Caregiver according to the plan of care (home visits)

Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner

Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources

Interprets the philosophy and objectives of the agency to clients, families, and others

Performs other job-related duties as may be required or directed by the supervisor

Required

Licensed Registered Nurse licensed by Missouri Board of Nursing

Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities

Valid driver's license with an insured vehicle for travel within the state

Strong communication and interpersonal skills

Problem-solving and decision-making abilities

Organizational and multitasking skills

Excellent communication, critical thinking, and organizational skills

Self-motivated

Able to thrive in a fast-paced start-up environment

Preferred

Bilingual in Spanish is a plus

Experience with CRM (Salesforce is a plus)

Home health or home community-based services experience

Start-up experience

Understanding of regulatory and provider compliance requirements
