Responsibilities
Drive new sales and referrals across Private Pay and MLTSS populations within the assigned territory
Own the full sales cycle: prospecting, needs-based consultation, service recommendation, referral conversion, and start-of-care
Conduct needs-based sales consultations with individuals and families to recommend appropriate home care services
Meet or exceed monthly and quarterly sales, referral, and census growth goals
Build and manage a robust pipeline of private pay leads and MLTSS referral opportunities
Develop and maintain relationships with MLTSS plans, care managers, service coordinators, discharge planners, and community partners
Expand volume within existing referral accounts through consistent follow-up, responsiveness, and service excellence
Generate referrals from satisfied clients, families, and professional partners
Represent the agency at community events, health fairs, provider meetings, and outreach activities
Deliver sales and educational presentations to providers, community-based organizations, senior housing, and professional groups
Qualification
Required
Proven sales or business development experience, preferably in home care, healthcare, or services
Experience selling or working within Private Pay and/or MLTSS environments
Strong relationship-building, follow-up, and closing skills
Familiarity with managed care, care coordination, or home care intake processes
Health Navigator, Care Coordinator, or Liaison experience
Willingness to travel Erie County and float to Monroe County when necessary
Preferred
Bilingual (English/Spanish)
