Responsibilities

You are required to effectively communicate horizontally and vertically across the organization, effectively articulating, writing, and expressing your concerns and interests to those who report to you, those who work parallel, and the executives within the organization
This position requires dynamic management skills to oversee, lead, and directly report findings and improvement opportunities related to services of third (3rd) party vendor partnerships, such as protective gear suppliers, laboratory services, and operational partnerships that directly correlate with infection control and prevention
You will be proficient in escalating your concerns, observations, and recommendations to the correct roles across the enterprise and throughout interconnected partnerships that influence your ability to create excellence in the infection control and prevention element of the operation
As fundamental infection control monitoring and prevention services are secured, you will work interprofessionally and within your department to innovate infection control by making the department, staff, and service points excellent and leading standards across the post-acute-care (PAC) space
You will lead, mentor, and manage infection prevention community level roles within the infection prevention department to ensure they perform at the top of their license, support the roles to create excellence, empower the roles to be successful, and professionally grow these roles to serve at a higher capacity if an opportunity becomes available
Fosters a culture of service excellence which anticipates and responds to the needs of our residents
Ensures and protects the confidentiality of sensitive information
You possess a strong clinical understanding of the impact infection control and prevention has on the PAC population with the skills to integrate workflow strategies into various softwares with the intent to maintain a legacy of your work and extraction of meaningful KPI’s from the systems you utilize in your routine work across the enterprise
Collaborates within the department to develop goals and objectives; motivate and manage care team members to accomplish the same
Collaboratively develops, implements, and evaluates the organizational infection prevention program to include policies and procedures for surveillance, epidemic and pandemic preparedness, hand hygiene, standard precautions, employee and resident infection control education, employee health, outbreak management, and transmission-based precautions
Establish enterprise and facility level routine, ongoing, and systematic collection, analysis, interpretation, and dissemination of surveillance date to identify infections, infection risks, communicable disease outbreaks and to maintain or improve resident health status
Utilize nationally recognized surveillance criteria such as but not limited to CDC’s National Healthcare Safety Network (NHSN) or Revised McGeer criteria to track trends and identify opportunities for improvement based on data analysis
Works within the team to develop an annual surveillance plan for the enterprise and individual communities based on the population(s) served, services provided, and analysis of surveillance data
Evaluate and modify the surveillance plan and infection prevention policies and procedures no less than annually and as new science, evidence, and best-practices are discovered
Provide infection prevention education to the region through written communication, oral presentations, recorded content, and other effective educational mediums that enhance the learners comprehension and retention
Consults with a team of infection preventionists to provide individual and small groups infection prevention education through written communication, oral presentations, recorded content, and other effective educational mediums that enhance the learners comprehension and retention
Serves as subject matter expert and resource across the enterprise, for individuals at the point-of-care as well as residents/families, partners, providers, visitors, community, and public health regarding infection prevention and control measures, including identification of infectious disease processes
Comply with regulatory and mandatory reporting requirements at the local, state, and federal levels and ensuring individual communities adhere to the highest level of transparency and reporting
Maintain current knowledge with CMS, state and federal regulatory agencies and public health guidelines as it relates to infection prevention
Participate in interdisciplinary performance and quality improvement process
Participate in the development/review of occupational health policies and procedures related to infection prevention and control
Reviews the risk of blood borne pathogens, OSHA standards, and exposure control plan to ensure implementation of best practice for occupational health to decrease the likelihood of exposures to infectious diseases
Develop (or assist with) immunization and screening programs i.e., Influenza and TB
Performs other tasks as assigned and within scope of practice

Required

Degree in nursing
Have primary professional training in nursing, medical technology, microbiology, epidemiology, or another related field
Is qualified by education, training, experience, or certification
Has completed specialized training in infection prevention and pursues new certifications and other credentialing for yourself and your direct reports
Collaboration, leadership, and conflict resolution
Communication skills oral and written
Ability to implement evidence-based guidelines
Knowledge in and ability to conduct data collection and analysis, report writing, and data presentation
