Responsibilities

Assist and support individuals in maximizing their everyday life by identifying individual’s needs, strengths, lifetime goals, and preferences in services/supports

Collaborate with individuals, families, and providers to ensure individuals are living their best life

Assist individuals in the development of their individual service plan, based on the results of assessments designed to develop outcomes appropriate to the individual's personal vision

Maintain regular communication and contact with individuals to ensure that all aspects of needed/necessary support are intact

Flexibility of time and work hours is necessary in this position

Experience in assisting individuals in the development of their individual service plan based on assessments

Ability to maintain regular communication and contact with individuals to ensure that all aspects of needed/necessary support are intact

Flexibility of time and work hours, including the potential need for evening and/or weekend hours
