Qualification
Required

Must be able to work in the field
Demonstrate excellent oral and written communication skills and the ability to listen effectively
Have the ability to work independently
Be a team player
Have the ability to organize and prioritize
Present a professional appearance and demeanor
Be able to attend occasional events as required
Have a valid driver's license and reliable transportation
Preferred
Prior knowledge of Maine area hospitals and familiarity with Medicare & MaineCare
Knowledge of the admissions/discharge process with respect to long term care facilities
Experience with NaviHealth referral management
