AccordCare is a licensed agency seeking experienced High-Tech Per Diem LPNs to provide skilled care throughout Broward County. The role involves delivering high-quality nursing and personal care, including specialized services for patients requiring ventilator/trach care, PEG tube management, and IV therapy.

Responsibilities

Bilingual Spanish/English preferred

Travel throughout Broward County, outside of Coral Springs as needed

Provide safe and effective personal care based on the individualized Plan of Care

Transport clients to and from appointments/activities safely as requested

Communicate promptly with Nurse Supervisor regarding changes in patient condition or emergencies

Deliver skilled nursing and personal care within scope of practice, including:

Medication administration

Transferring and positioning

Catheter care

Communication with client, family, and Nursing Supervisor

Occasionally perform client assessments and supervisory visits

Demonstrate empathy, professionalism, attention to detail, and strong follow-through

Anticipate potential issues and appropriately escalate or resolve concerns

Prioritize tasks and respond with a sense of urgency
