Responsibilities
Assess, plan, implement, and evaluate nursing care for individuals who are well, chronically ill, or acutely ill
Implement and oversee individualized care plans focused on health maintenance and disease prevention
Coordinate communication and care activities across interdisciplinary healthcare teams
Manage medication processes, including ordering, monitoring, documentation, storage, and disposal in accordance with policy
Communicate changes in patient condition to physicians and ensure accurate transcription of medical orders
Maintain required daily, monthly, quarterly, and annual nursing documentation
Supervise and direct licensed practical/vocational nurses as needed
Schedule and follow up on health-related appointments to ensure timely care
Collaborate with inpatient and discharge coordinators during hospitalizations
Provide training, onboarding, and annual recertification for staff on health and safety topics
Ensure infection prevention standards are maintained for equipment, supplies, and environments
Qualification
Required
Education: Associate's or Bachelor's degree in Nursing (or related field)
Attendance: Commitment to mandatory attendance during the first 90 days
Active, unrestricted Registered Nurse license in Georgia
Ability to work independently in a community-based setting
Strong clinical judgment and documentation skills
Willingness to participate in an on-call rotation
Must have a reliable vehicle and active auto insurance (non-negotiable; daily travel required)
