Patient Navigator Specialist

They are seeking a Patient Navigator Specialist who will be responsible for receiving, assessing, and resolving patient grievances and concerns while collaborating with various teams to ensure timely resolutions.

Responsibilities

Receive, investigate, document, and resolve patient grievances and complaints in a timely and professional manner
Advocate for patients and families while ensuring compliance with hospital policies, Patient Rights & Responsibilities, and regulatory standards
Serve as a liaison between patients, families, clinical staff, and hospital leadership
Analyze grievance data to identify trends, risks, and opportunities for quality improvement
Maintain accurate documentation and reporting in accordance with Joint Commission and CMS requirements
Apply strong organizational knowledge to identify appropriate resources and escalate issues when necessary
Work effectively with ethnically, culturally, and sexually diverse populations using trauma-informed and culturally competent approaches

Required

Direct experience handling patient grievances or working in patient advocacy within a hospital or healthcare system
Minimum of three (3) years of relevant experience in healthcare, patient relations, public health, compliance, or equivalent hands-on experience
Demonstrated experience working with ethnically, culturally, and sexually diverse populations
Knowledge of HIV/STD treatment and prevention with an emphasis on harm reduction principles
Strong working knowledge of: Patient Rights & Responsibilities
Joint Commission standards
Centers for Medicare & Medicaid Services (CMS) regulations
Proficiency in medical terminology
Strong analytical skills, including data collection, compilation, and analysis
Proven ability to assess grievances quickly and determine appropriate resolution pathways
Bachelor's degree in a related field and three (3) or more years of relevant experience OR An equivalent combination of education, training, and hands-on experience
