Responsibilities
Conduct thorough assessments of patients’ behavioral health needs and develop individualized care plans
Coordinate with healthcare professionals and community resources to ensure continuity of care
Monitor patient progress and update care plans accordingly to reflect changes in condition or treatment goals
Facilitate communication between patients, families, and care teams to support patient-centered care
Document all case management activities accurately and maintain compliance with hospital policies and regulatory requirements
Assist patients in accessing necessary services such as housing, counseling, and medication management
Participate in multidisciplinary team meetings to discuss patient care strategies and discharge planning
Provide education and support to patients and families regarding behavioral health conditions and treatment options
Qualification
Required
Valid Licensed Vocational Nurse (LVN) license in the state of California
Minimum of 2 years of experience in behavioral health nursing or case management
Basic knowledge of behavioral health disorders and treatment modalities
Strong understanding of patient confidentiality and HIPAA regulations
Proficiency in electronic medical records (EMR) systems
Preferred
Certification in Case Management (CCM) or equivalent
Experience working in an inpatient behavioral health unit
Familiarity with community mental health resources and social services
Advanced training in crisis intervention and de-escalation techniques
Bilingual abilities, particularly in Spanish, to better serve the local community
