Care Management Support Coordinator II McAllen
Dean's Professional Services is actively seeking a Care Management Support Coordinator II to support care management operations for a large healthcare organization. This fully remote role offers consistent weekday hours, meaningful member interaction, and the opportunity to contribute to care coordination, compliance, and service delivery across a collaborative team environment.

Responsibilities
Conduct outbound outreach calls to members, providers, and community agencies
Serve as a front-line point of contact for inbound member and provider inquiries
Support member onboarding by sending welcome letters, correspondence, and educational materials
Review assessments, service plans, and follow up on referrals, hospital discharges, and pending services
Assist with long-term services and supports (LTSS), including authorizations, updates, and member education
Research and connect members to health plan and community resources, including SDOH support
Support utilization management activities and follow up on denials or requests for additional information
Accurately document and maintain non-clinical member records in compliance with regulatory standards
Assist with escalations, complaint research, and reporting to leadership
Perform additional administrative and care support duties as assigned
Qualification
Required
High School Diploma or GED
1-2 years of healthcare, care coordination, administrative, or customer service experience
Strong computer skills with the ability to navigate multiple systems efficiently
Excellent communication, organization, and time-management skills
Ability to work independently in a remote environment
Preferred
Medical Assistant experience
bilingual (English/Spanish) skills
