Responsibilities
Assess nature of clients' situations by interviewing and reviewing personal history
Develop and execute individual treatment plans
Determine appropriate milestones to gauge client progress Communicate client updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs Maintain accurate client documentation
Crisis assessments/triage with appropriate interventions
Provision of other services related to wellness
Other related activities designed to provide integrated health care services
Qualification
Required
Master's Degree
LCSW (New York license)
one to two years' experience providing clinical services in the mental health field
Bilingual English/Spanish
Medicaid and Medicare Enrollment is must!
