Care Management Support Coordinator II
Dean's Professional Services is actively seeking a Care Management Support Coordinator II to join a collaborative care management team supporting member services and compliance initiatives. This role involves conducting outreach, managing member inquiries, and supporting long-term services and supports, all while ensuring compliance with regulatory standards.

Responsibilities
Conduct outbound outreach to members, providers, and community agencies
Serve as a front-line point of contact for inbound calls and member/provider inquiries
Review assessments and service plans and follow up on referrals, hospital discharges, and pending services
Support Long-Term Services and Supports (LTSS), including authorizations, updates, terminations, and member education
Research and connect members to community and health plan resources, including Social Determinants of Health (SDOH)
Assist with utilization management activities, including denials and requests for additional information
Support complaint research, escalations, and reporting to leadership
Accurately document and maintain non-clinical member records in compliance with state and regulatory standards
Participate in audits and performance reviews to ensure turnaround time and compliance requirements are met
Perform additional administrative care support duties as assigned
Qualification
Required
High School Diploma or GED (required)
1-2 years of healthcare, care coordination, administrative, or customer service experience
Strong computer proficiency and ability to navigate multiple systems
Excellent communication, organization, and time-management skills
Ability to work independently in a remote environment
Preferred
Medical Assistant background
bilingual (English/Spanish) skills
