In this role, you will lead care teams dedicated to providing high-quality hospice services and develop relationships within the community while ensuring excellent clinical and social care.

Support the team by providing clinical guidance and judgment to promote understanding of and response to patient and family needs
Participate in Performance Improvement & Quality Assurance activities
Develop and mentor future hospice leaders
Qualification
Required
Passion for focusing on hospice care (Former Hospice experience required)
Desire to lead an interdisciplinary care team
Preferred
Registered Nurse, BSN or Masters preferred
Management experience strongly preferred
