Responsibilities

Develop and maintain referral relationships with cardiologists, hospitals, case managers, discharge planners, and physician offices across the LA market
Promote home health care services, emphasizing cardiac care and chronic disease management programs
Consistently achieve monthly and quarterly admissions and referral targets
Educate referral sources on clinical outcomes, patient benefits, and regulatory compliance
Monitor market trends, competitor activity, and payer mix
Collaborate with clinical and operations teams for seamless patient transitions
Document all sales activities and referral data in CRM systems

Qualification

Required

3–5 years of home health or healthcare marketing experience in the Los Angeles market

Valid driver's license and reliable transportation

Preferred

Proven success working with cardiology practices or cardiac-related services
Bachelor's degree preferred (Healthcare or Business-related field a plus)
