Home Care Planner
MJHS is a large not-for-profit health system in the Greater New York area. In this role, you will evaluate patients referred for home health services and manage their transition onto the home care program in accordance with applicable laws and agency policies. Responsibilities include establishing care plans, communicating with healthcare teams, and managing the referral process to optimize patient care.

Meets the established Monthly Admission Targeted Goals
Provides flexibility within the role of effectively supporting the agency’s overall business objectives and goals
Establishes a plan of care (POC) in collaboration with patients, physicians and members of interdisciplinary health care team and arranges for all needed services under the direct supervision of an HCP Manager and/or Director of Business Development
Works in conjunction and maintains consistent communication with discharge planning staff to identify problems that may impede the transition of the patient returning home and assists with resolution as needed; under the direct supervision of an HCP Manager and/or Director of Business Development
Communicates agency admission criteria, reimbursement criteria and policies to patient/caregiver, members of the interdisciplinary health care team and other personnel as indicated; under the direct supervision of a HCP Manager and/or Director of Business Development
Conducts an evaluation of referral needs to determine potential alignment with additional MJHS products and services (Elderplan, HomeFirst, Hospice, Palliative Care); under the direct supervision of an HCP Manager and/or Director of Business Development
To identify and assess referrals received for the various MJHC clinical service programs (e.g. Heart Failure, COPD, Joint Replacement, Diabetes Management, Community Based Palliative Care) as deemed applicable; under the direct supervision of an HCP Manager and/or Director of Business Development
To identify and assess that the Home Health Face-to-Face documentation as mandated by the Centers for Medicare and Medicaid Services (CMS) is in all Medicare patient’s medical record under the direct supervision of an HCP Manager and/or Director of Business Development
Manages the referral process, ensuring timely and accurate coordination between departments to optimize patient care and streamline operations; under the direct supervision of an HCP Manager and/or Director of Business Development
Qualification
Required
Associates in Nursing
Minimum of one year of acute, Medical-Surgical nursing experience
Specialized knowledge in Microsoft Office, (Word, Excel, Outlook, etc.)
Preferred
Baccalaureate Degree
Home Care experience
