Board Certified or Board Eligible in Family Medicine or Internal Medicine
If Board Eligible, graduation date must be provided
Active Vermont medical license (or ability to obtain before start)
DEA certification; BLS certification
Clean malpractice history
Commitment to full-time, outpatient practice
