Responsibilities
Coordinate and manage clinician schedules to ensure optimal patient coverage
Communicate with patients, caregivers, and clinical staff to confirm visits and address scheduling changes
Collaborate closely with Covenant leadership and Concierge clinical teams to support daily operations
Assist with workflow coordination to ensure continuity and quality of care
Provide hands-on nursing care to patients in their homes under the direction of a Registered Nurse
Follow individualized, goal-driven plans of care established by the RN
Deliver skilled nursing services, including:
Acute and chronic disease management
Medication administration and management
Post-surgical support
Pain management
Wound care
Complete timely and accurate documentation using streamlined tools and dictation services
Qualification
Required
Unencumbered Florida Licensed Practical Nurse (LPN) license (required)
Valid driver's license, reliable transportation, and active auto insurance (required)
Strong organizational, communication, and time-management skills
Preferred
Home health experience preferred, but not required
