Program Coordinator (Supervisory) - Madison
Phoenix - Huntsville Rehabilitation Foundation, Inc. is seeking a Program Coordinator (Supervisory) to oversee program activities and ensure compliance with policies and practices. Responsibilities
Administer the department in compliance with Phoenix procedures, relevant regulatory guidelines, and program goals
Be familiar with the procedures of ADRS, such as those about facility services and current fee-for-service requirements, CARF standards, DOL standards, and the standards for any other agency as might be appropriate to ensure that the department and its services comply with all applicable standards
Be responsible for ongoing and annual written job performance evaluations on each employee supervised. Make work assignments as appropriate and ensure staff coverage of the department
Monitor the routine functioning of the department, including reviewing staff requests for equipment, approval of supply purchases, approval of staff requests for leave, approval of timesheets, monitoring appropriate departmental reports and correspondence, and ensuring equipment maintenance as required
Develop and maintain a program manual/job book defining departmental goals and objectives, departmental services, criteria for admission to the department, departmental intake procedures, development of individualized programs of service, maintenance of client records, and reporting procedures for each program area supervised
Provide information to the Vice President of Vocational Programs to fulfill overall facility monthly, quarterly, and annual reporting requirements
Work with Phoenix management personnel in carrying out an ongoing public relations program
Provide employment readiness training (group and individual), assessment, job development, job placement, and follow-up for referred persons as needed to augment overall departmental capacity
Serve as a facility consultant regarding the employment readiness of program participants
Serve as the primary resource person for information on current employer needs
Develop new and innovative ideas for the delivery and improvement of service, being mindful of the needs of persons with disabilities and the agencies that refer them, as well as the demands of local employers
Serve as an active member of the vocational services management team to participate in decisions affecting the department, such as the establishment of departmental goals and objectives, financial management, staffing, space allocation, consumer selection, in-service training, consultant utilization, public relations, and program development
Represent Phoenix in the business community by involvement in civic and professional organizations
Provide information as requested for various statistical requirements
Maintain competencies as defined by the Phoenix Professional Development Plan
Provide services in response to referral source needs and regarding internal financial requirements promptly
Be actively involved in all marketing exercises as requested
Provide case management services to individuals with disabilities, including service coordination and referrals to social service agencies within the community
Maintain contact and communication with appropriate vocational services staff to provide effective case management services
As necessary, develop, write, and present progress reports and discharge summaries documenting performed activities
Qualification
Required
Master's degree from an accredited college in rehabilitation, psychology, social work, sociology, counseling, or a related discipline (a combination of equivalent education and work experience may be considered)
Minimum of three years' experience in a rehabilitation, non-profit facility, or comparable setting in the area(s) of vocational rehabilitation, mental health counseling, service delivery, or case management
Proficiency with the Microsoft Office suite and general computer operation is required
Working knowledge of the broader rehabilitation movement, CARF, Alabama Department of Rehabilitation Services, ADA, and relevant DOL guidelines
Strong working knowledge of disability management and awareness
Knowledge of the AbilityOne federal law
Must be authorized for coverage under agency insurance if operating an agency vehicle, have a good driving record, a valid driver's license, and/or the required level of personal auto insurance
Must be able to satisfactorily complete a background check, per company requirements
Ability to communicate with tact and maintain confidentiality
Adequate ability in communicating in spoken conversation – adequate volume of speech
Adequate ability in writing/reading/mathematics
Ability to effectively build and maintain positive working relationships with management and peers
Strong organizational, project planning, and management experience with proven analytical and multi-tasking skills
Superior listening, verbal, and written communication skills
Proficiency in general reporting and basic case file management
Preferred
Ability to supervise and manage report requirements and perform home visits
Professional Certification or equivalent certification preferred: Certified Rehabilitation Counselor, Licensed Professional Counselor, or Licensed Clinical Social Worker
