Responsibilities
Finding resources to assist the patient with: transportation, obtaining medications, financial barriers, social security or employee-based disability claims, or other barriers to safe and successful treatment
Collaborates with behavioral health team to meet the needs of high risk populations
Helps to facilitate care transitions and other specialties when needed
Develops and maintains community relationships to support client referrals
Consults and collaborates with other professionals involved in providing services to patients
Attends ROSC-related meetings in or out of the office including with psychiatric and medical prescribers as necessary
Documents and provides data entry for grant reporting and tracking for services delivered through grant funded programs
Keeps the director/manager informed of problematic cases, especially those involving legal or risk management issues
Qualification
Required
Driver's License - Illinois Secretary of State (ILSOS)
Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
Ability to problem-solve and develop solutions for patients and their families
Strong knowledge of available community resources
Awareness of when and how to access additional resources to meet a patient's needs
Strong advocacy skills necessary
Work Experience: Related field
