This role involves providing outpatient physical therapy services to adults with Intellectual and Developmental Disabilities, focusing on enhancing mobility, function, and independence.

Responsibilities
Conduct evidence-based evaluations and develop individualized treatment plans to enhance mobility, function, and independence
Establish goals that respect each patient’s dignity, culture, and unique needs
Work closely with medical providers, caregivers, and families to ensure coordinated care
Complete accurate and timely documentation in the EMR system (paid administrative time provided)
Educate patients and support staff on home exercise programs, safety strategies, and functional skills for daily living
Qualification
Required
Current and valid New York State Physical Therapist license
Strong clinical judgment
Clear documentation skills
Empathetic communication skills
Preferred
Experience working with adults with I/DD
