Care Transition Specialist (Administrative) Full time
Texas Health Resources is a healthcare organization dedicated to improving community health. They are seeking a Care Transition Specialist to manage clerical and administrative processes for care transitions, support transition planning activities, and handle communication with vendors and clinics.

Responsibilities
Responsible for coordinating the clerical and administrative processes for entity care transitions management
Responsible for managing departmental mailbox, phone lines and/or fax
Supports transition planning activities as needed under the direction of care transition managers
Calls vendors, physician offices, clinics, etc
Faxes (electronic and traditional) documents as needed
Makes copies or gathers other data as needed
Collect and enter data as needed
Completes other tasks and projects assigned
Qualification
Required
H.S. Diploma or Equivalent required
Three years' relevant experience, preferably in a healthcare or service environment required
MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software
Preferred
Patient Care/ Customer Service /Healthcare experience strongly preferred
