Responsibilities
Assess member needs, available resources, and recommend or facilitate care plans for optimal outcomes
Develop and support ongoing long-term care service plans
Identify and coordinate providers, specialists, and community resources
Serve as a liaison between members, families/caregivers, and care teams
Provide members and caregivers with resources related to employment, housing, independent living, justice involvement, foster care, and participant direction
Monitor member status, care plans, and outcomes; recommend adjustments as needed
Collaborate with healthcare providers and community partners to ensure services meet member needs
Conduct on-site visits to assess member needs when appropriate
Educate members and caregivers on care procedures, provider instructions, service options, referrals, and healthcare benefits
Maintain accurate documentation of care management activities in compliance with state, federal, and payer regulations
Provide feedback to leadership on opportunities to improve care quality and cost-effectiveness
Qualification
Required
Bachelor degree
Experience working with Medicaid and special health care needs
must have experience working with: Addictive and dysfunctional family systems
must have experience working with: Racial and ethnic minorities
must have experience working with: Homosexuals and bisexuals
must have experience working with: Persons with AIDS
must have experience working with: Substance abusers
