Responsibilities
Engage a vendor to support its integrated care management, disease management and utilization management functions within the CCA platform
Create and maintain care models that match the required plans to support the related processes for assessments, care planning, member education, and documentation and tracking
Create and maintain clinical criteria along with integrations with external clinical criteria vendors for consolidated clinical reviews
Update and automate the care flows required to include counters, notifications, advancing functional care plans
Create and maintain advanced operational reporting and dashboards to be used for identifying gaps in service and overall performance
Support required integrations with Samaritan Health System EMR's, provider portals, etc
Qualification
Required
Expertise in Utilization Management (UM) and Care Management (CM)
Experience in configuration, maintenance, and reporting of the CCA platform
Ability to inject automation to create functional care plans with fewer touches
Experience in creating and maintaining care models that match required plans
Experience in creating and maintaining clinical criteria and integrating with external clinical criteria vendors
Experience in updating and automating care workflows including counters and notifications
Experience in creating and maintaining advanced operational reporting and dashboards
Experience in supporting integrations with EMR systems and provider portals
Ability to propose alternate UM/CM platforms and provide total costs for alternatives
