2+ years expYWCA Greater Harrisburg is seeking a passionate and dedicated full-time Transitional / Rapid Rehousing Case Manager to join their team. This position works directly with individuals and families experiencing homelessness to develop service plans, connect them to resources, and guide them in securing safe housing.

Responsibilities
Conduct client intakes, assessments, and individualized goal planning
Provide weekly case management sessions and ongoing advocacy, including referrals and support at external appointments
Assist clients in securing housing, understanding leases, and meeting tenant responsibilities
Deliver crisis intervention and support to promote client safety and housing stability
Maintain accurate client records, data entry, and required monthly/quarterly reporting
Collaborate with property management to ensure compliance with housing and program requirements
Build relationships with community partners and local landlords to expand housing access
Conduct exit interviews and complete required documentation when clients transition from the program
Participate in meetings, trainings, and the program’s on-call rotation
Perform additional duties as assigned
Qualification
Required
Bachelor's degree in human services or related field with two years' experience or associate degree equivalent with four years' experience
In-depth knowledge and understanding of the social welfare system
Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required
Current FBI, criminal, and child abuse clearance or the ability and willingness to obtain
