Information and Assistance Specialist

Responsibilities

Assists by providing information through telephone, written form, and office visits. Explains community services, government benefits, philosophy, and consumer issues
Refers clients to appropriate services, including those provided by other organizations and those provided by Passages (e.g., Ombudsman, HICAP, Senior Companion)
Visits with seniors, significant family members, and caregivers who walk in or make appointments at the office in order to analyze client concerns, provide information, and generate options for client consideration. Assists in implementing a plan of action if the client is unable to do so independently. Advocates on behalf of the client, if appropriate
Participates in ongoing efforts to research and organize community resources, including the Passages’ resource informational database and web page. Maintains thorough personal knowledge of available services and eligibility requirements for government benefits such as Medi-Cal and Social Security
Collects and compiles service and client data; updates client information in database
Works cooperatively with other agencies and organizations to aid seniors, adults with disabilities, and their caregivers
Disseminates information about Passages’ services, consumer issues, government benefits, and long-term care issues through public speaking, attendance at public meetings, and other outreach forums provided by Passages
Receives Ombudsman complaints from facilities and consumers and transfers information to Ombudsman staff and volunteers as appropriate
Effectively articulates the ADRC philosophy in daily tasks by facilitating warm transfers to the ADRC partner, the Disability Action Center. Responsible for completing the initial intake form and offering options counseling to clients, their close family members, and caregivers regarding available community resources
Provides support to the Senior Nutrition Department by conducting, maintaining, and organizing initial, quarterly, and annual client assessments and department communications. Completes other assessments as required
Provides services using tact and courtesy when dealing with callers who may be experiencing difficult conditions or situations. Demonstrates the ability to remain patient and understanding

Required

Candidate must be a California resident
Bachelor's degree in Social work, Community Services, or a related field preferred
Position requires the ability to drive to occasional community meetings and health fairs
Satisfactory completion of a background check (including a criminal records check) is required for employment
Candidate must be in possession of valid driver's license and automobile liability insurance
Participation in the DMV Employer Pull Notice Program (driving record) is required
