Responsibilities
Maintains up to date knowledge base of PrEP, U=U, HIV/HCV/STI transmission, treatment and disease progression, interpretation of HIV/HCV test results and other related STI/HIV/HCV issues. Additionally, conducts and evaluates clients' health education sessions as related to HIV/HCV prevention and risk reduction; provides HIV/HCV counseling and testing and follow-up with clients
Links patients to HIV care and/or PrEP services. Participates in staff development meetings, representing prevention, education and outreach issues, activities, goals, and outcomes
Leads social media/campaign efforts to publicize CCHIP community HIV testing events and PrEP Services. Maintains and monitors social media platforms and CCHIP website in compliance with CCH rules and standards
Works closely with the Director of Community Affairs and CCH Communications Department to inform system of HIV/PrEP services/events
Works with CBO partners to coordinate Mobile Testing Unit (MTU) HIV rapid testing and community outreach. Establishes and schedules MTU days for CCHIP sites
Coordinates community forums with local stakeholders & Ball Room community regarding PrEP awareness & education
Initiates immediate contact with patients/participants who are interested in PrEP to establish rapport and assist with linkage to ongoing PrEP services; completes required assessment, MAP applications, and/or intake information for follow-ups and enters data in required database(s)
Manages CCHIP Event Log and Report out to the Director of Community Affairs monthly
Develops community relationships and work with Specialty Clinic Coordinator to identify linkage agreements
Reviews and assess current community outreach and testing services/procedures to implement and standardize best practices where possible
Assists in the creation new procedures at sites where community outreach and testing services are not efficient or need improvement
Works with the CCHIP Program Director and Project Management Administrator and CCHIP leadership to improve community outreach and testing services and better utilize existing resources
Collaborates with CCHIP Director of Quality Improvement and Evaluation, CCHIP Program Director and CCHIP team leaders to assess program effectiveness in achieving stated goals as required by funding agency
Coordinates inclusion of Peer Educators in community testing events and provides or assigns onsite supervision assigned Peers
Conducts Quarterly POC Testing refresher trainings, and HIV Testing Certification trainings for CCHIP staff
Documents all face to face, phone calls, and other encounters related to HIV testing and PrEP Services in the EMR (Electronic Medical Record), and other databases required by funders
Documentation of all client-related activities, referrals, including approvals and denials should be entered in EMR and other required databases
Documents all data entry and paperwork will be submitted in compliance with organization's policies
Documents all transportation, food vouchers, transportation, and cab vouchers must be properly documented including client's signature of receipt
Participates in quality improvement activities
Qualification
Required
Highschool diploma or equivalent
Two (2) years experience working in a healthcare setting as a health educator; working with diverse populations, including LGBTQ+
Experience with multiple HIV rapid test technologies
IDHPH or CDPH HIV testing certification
Valid Driver's License and Insurance or other means of reliable transportation
Must be able to work nights or weekends as requested
Preferred
Bachelor's degree from an accredited college or university
