Quality Analyst

Responsibilities

Responsible for assigned schedule of in person visits to all assigned group homes/day programs
Assigned schedule of unannounced evening home visits
Accurately assess and document performance of assigned group homes/day programs
Assist group home/day program staff with remediation of risks and subpar performance
Conduct in-depth audits on all assigned group homes/day programs
Incident management:
Reviews clinical assessment and reassessment processes to mitigate risks unique to program participants
Assesses the effectiveness of risk management systems within the program using baseline data
Conducts systemic risk assessment reviews at least annually to identify and respond to practices, situations, and policies that could result in the risk of harm to individuals receiving services
Reviews and updates the Company’s written Risk Management plan at least annually based on ongoing quality review and risk management activities
Reviews staff competency and adequacy of staffing to meet needs of program participants
Establishes quality control standards and parameters through a Quality Improvement Plan and QA audits; continually monitors quality and report results
Conducts individual screening, investigations, root cause analysis, and uses data to analyze trends and patterns
Reviews all program incident reports and completes follow up and/or action plan
Submits incident reports to all necessary parties within a specified time frame in accordance with state and agency requirements
Complete follow-up investigations and interviews on incidents as needed and provides a final report to include an action plan for follow-up under the direct supervision of the Program Director, or designee

Required

Highschool diploma or GED required
Minimum 3 years' experience in group home/day programs and/or in DSP role
Excellent verbal and written communication skills
Self-Starter with the ability to work with little supervision

You will be responsible for learning and becoming proficient with these IT systems and others that BSLS may deploy or change in the future
