Licensed Practical Nurse – Consultative Medicine General Internal Medicine - LPN

Responsibilities
Patient visits (face to face and via telephone)
Review of patient health history
Medication review
Immunization administration
Focused nutrition assessments
Blood pressure visits
Collecting pertinent information prior to their arrival to the clinic
Researching prescription discrepancies
Facilitating prior authorization denials
Provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor
Accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family
Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning
Participates as a member of the health care team
Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care
Possesses excellent communication skills
Is skillful in mentoring and instructing
May participate on committees or projects, including quality improvement projects
Qualification
Required
Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program
If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required
Computer skills required
Ability to work daytime hours, flexibility may be required to meet staffing needs
Ability to adapt to unpredictable situations within the work setting
Able to work effectively with multiple members of the interdisciplinary care team
Demonstrates strong, effective and open communication skills to achieve mutual understanding and build trust and rapport with team members
Able to work in a rapidly changing environment while participating in developing innovative and creative solutions to patient care
Excels in creative experimentation and is able to explore new paths, procedures, and/or approaches to models of care teams
Displays flexibility in adapting and coping with accelerated changes within the environment
Current RN license by applicable state requirements
BLS (Basic Life Support) must be valid for 90 days post start date; acceptable certifications include: American Heart Association (AHA)/ Heartcode® BLS, Military Training Network, Red Cross (CPR/AED for Professional Rescuers and Health Care Providers)
Additional state licensure(s) and/or specialty certification/training as required by the work area
Preferred
One year LPN experience within the last five years preferred
Experience working in a team environment
Prior experience with electronic medical record systems preferred
Graduate of an accredited school of practical nursing preferred
