Responsibilities
Perform comprehensive clinical assessments of hospice patients
Develop, implement, and continuously reassess individualized care plans
Collaborate with physicians, social workers, discharge planners, and other care team members
Coordinate transitions of care to appropriate levels and settings
Identify and arrange non-medical supports (e.g., transportation, housing) to promote treatment adherence
Monitor clinical, functional, and fiscal outcomes and maintain detailed documentation
Engage specialty resources as needed to optimize patient outcomes
Identify opportunities for health promotion and illness prevention
Proactively prevent adverse patient events and intervene promptly when needed
Act as a patient advocate while ensuring privacy and confidentiality
Qualification
Required
Education: Associate's or Bachelor's degree in Nursing or related field
Attendance: Commitment to mandatory attendance during the first 90 days
Active, unrestricted Minnesota RN license
Hospice experience (required)
Strong clinical judgment and ability to analyze complex medical information
Excellent communication and care coordination skills
