Rural Health Transformation (Health Information Technology) Hybrid (Durational Project Manager) #251229-0415VR-001
The State of Connecticut is seeking two Durational Project Managers to support its Rural Health Transformation Program (RHTP). These roles will lead the implementation of health information technology interventions aimed at improving healthcare access and delivery in rural communities, focusing on areas such as provider shortages and fragmented care coordination.
Government Administration
Responsibilities
Provide overall leadership and accountability for Rural Health Transformation HIT initiatives, from planning through implementation and delivery
Translate high-level policy goals into actionable program plans, milestones, and deliverables
Develop and maintain comprehensive project plans, including timelines, dependencies, performance metrics, and reporting frameworks
Manage scope, prioritize workstreams, and evaluate change requests to ensure alignment with program goals
Oversee operational aspects of the federal grant, including reporting, monitoring, evaluation, and financial and project management activities
Identify, assess, and mitigate program risks and issues, including operational, technical, policy, and stakeholder risks
Serve as a primary liaison with partner state agencies (including DSS, DPH, and OPM), healthcare providers, vendors, and external stakeholders
Support collaboration and adoption among rural healthcare partners and other key stakeholders

Preferred
Experience utilizing Health Information Technology, such as HIE, EHRs, data integration, telehealth, or other HIT experience
Experience managing health policy, healthcare administration, public-sector programs, or health system or healthcare administration experience
Experience with rural health and service delivery
Experience managing complex, multi-stakeholder initiatives
Experience utilizing health data for analytics and performance measurement
