This role is central to ensuring smooth patient intake, timely scheduling, and seamless coordination between patients, nurses, providers, and internal leadership.

Responsibilities
Receive and process new patient referrals
Collect and verify patient demographics, insurance information, and required documentation
Communicate with patients and families regarding next steps, scheduling, and expectations
Coordinate start-of-care visits and ongoing services
Schedule home health and home infusion visits
Communicate assignments and updates to nursing staff
Adjust schedules as needed due to patient or nurse availability
Support coverage coordination and last-minute changes
Maintain accurate and up-to-date patient records in the EMR
Track authorizations, orders, and required documents
Support leadership with operational tasks as needed
Assist with internal process improvement and workflow organization
Serve as a key point of contact between patients, nurses, providers, and leadership
Communicate clearly, professionally, and compassionately
Escalate clinical or operational concerns appropriately
Ensure documentation completeness and accuracy
Support compliance with agency policies and regulatory requirements
Assist with chart preparation and audits as directed
Qualification
Required
Certified or experienced Medical Assistant
Prior experience in healthcare coordination, clinic operations, home health, or similar settings
Strong organizational and time-management skills
Comfortable using EMRs, scheduling systems, and basic office technology
Excellent written and verbal communication skills
Ability to work independently and manage shifting priorities
High level of professionalism, discretion, and reliability
Preferred
Experience in home health and/or home infusion
Familiarity with insurance verification and authorizations
Experience working remotely in a healthcare setting
Comfort supporting a growing startup-style organization
