Responsibilities
Serve as a subject matter expert for HEDIS® and quality improvement measures
Conduct provider and member outreach to close gaps in care
Facilitate meetings with providers and internal stakeholders
Plan and execute outreach workplans using data and analytics
Support medical record collection and abstraction
Deliver education and presentations to internal and external partners
Identify and implement process improvement opportunities
Data analytics and presentation
Qualification
Required
3+ years of healthcare experience
1+ year of provider or member education experience
HEDIS® and quality improvement knowledge
Proficient knowledge of Microsoft applications excel, word and outlook
Strong written, verbal, and presentation skills
Ability to work independently in a metric-driven environment
Preferred
Bachelor's degree or equivalent healthcare experience
Certified Health and or Wellness Coach
Experience working with medical providers or office staff
Knowledge of medical coding
Managed care or health insurance experience
