Program Manager
Housing Works is a nonprofit organization committed to fighting for funding and legislation for people living with HIV/AIDS. The Program Manager is responsible for overseeing service delivery to clients within the CCR program, managing day-to-day operations, and supervising staff while collaborating with a multidisciplinary care team.

Responsibilities
Responsible for monitoring the service-based aspects of the contract
Provide supervision to the department staff, including Navigators and Outreach Workers
Reviews/approves assessments, reassessments, care plans, PCSMs, self-management assessments, and case conferences
Monitors programs’ enrollment and discharge trends to ensure retention and quality
Maintains staff scheduling to ensure adequate program coverage at all times
Collaborate with key stakeholders at each site, including leadership clinic staff, program staff to ensure effective communication and service delivery
Ensure all client records are maintained in accordance with all state and federal laws, rules and regulations
Participates in the development and execution of Quality Improvement Initiatives in collaboration with the department and QI Team
Ensure high-quality client experience by ensuring all client care is coordinated across Housing Works programs and addressing client issues and barriers to care as necessary
Other duties that support programmatic goals as needed
Participation in Housing Works supported advocacy events
Qualification
Required
College degree required
Experience with program management
Knowledge and experience with the dually diagnosed community of HIV/AIDS and mental illness
Knowledge and experience with state and local policies of NYC affecting persons living with mental illness, HIV/AIDS
Some Experience in program planning and program operations
Experience working with homeless populations and active substance users
Ability to supervise, manage and provide leadership to all staff members and trainees
Demonstrated ability to work effectively independently, manage a team, and collaborate across departments, required
Excellent written and verbal communication skills, required
Strong managerial skills including communication, motivating employees, problem solving, and progressive discipline
Commitment to maintaining the highest levels of confidentiality
Demonstrated ability to build the trust and respect of patients, staff, colleagues, external contacts, and other Housing Works members
Sensitivity to needs of culturally and linguistically diverse patient and employee population
Demonstrated ability to meet or exceed the Service Excellence Standards of Housing Works
Willingness and ability to work some evenings and weekends, as needed
Demonstrated ability to utilize electronic medical records (EMRs)
