Responsibilities
Serve as the attending physician for residents in each assigned SNF
Collaborate with onsite NPs/PAs to manage daily patient care and oversee clinical supervision
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions
Complete timely documentation in ChartEasy™ and work with facility EHRs (PointClickCare, MatrixCare, AHT)
Participate in Theoria’s Chronic Care Management, Transitional Care, RPM, and other quality initiatives
Utilize electronic medical systems, including EHRs and state/county tools for clinical and administrative tasks
Qualification
Required
MD or DO degree from an accredited medical school
Valid medical license in the state of Tennessee
Board certification in a relevant specialty
Experience in primary care or post-acute care settings
Ability to collaborate with NPs/PAs and manage patient care
Proficiency in using electronic medical systems, including EHRs
Strong communication and interpersonal skills
Preferred
Experience with ChartEasy™ or similar EHR systems
Familiarity with skilled nursing facilities and post-acute care
Interest in chronic care management and quality initiatives
