Clinical Pharmacist Specialist (Practitioner)
The VA Maine Healthcare System is seeking a Clinical Pharmacist Specialist at the Togus VA Medical Center. This role involves providing clinical and teaching services, including direct patient care and medication management for Veterans with chronic diseases.

Responsibilities
Responsible for the evaluation, monitoring and medication management of Veterans referred by consult to the Clinical Pharmacy Specialist for chronic disease state management
Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications
Completes all encounters, charting and documentation including scheduling, reviewing consults and finishing encounters, to meet timeliness standards
Participates and/or supports Pharmacy Service-initiated cost/benefit programs in support of VA Maine Healthcare's efforts at maintaining stewardship of resources
Provide follow-up on reported adverse drug events in the ambulatory care and inpatient medical/surgical patient care areas or other areas within the medical center when necessary
Attends and actively participates in clinical pharmacy activities, i.e., staff development, clinical pharmacy meetings, journal club
Provides in-service educational programs (written or verbal) to selected Pharmacy Service and medical center staff and providers at the direction of the Clinical Coordinator and/or Chief of Pharmacy
Qualification
Required
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Physical Requirements: See VA Directive and Handbook 5019
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
Must be proficient in written and spoken English
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
Preferred
Experience in Ambulatory or Primary Care settings. This experience should include direct patient care and comprehensive medication management, and could be from residency training
Ideal candidates will have a mix of the following qualifications: PGY1 residency, PGY2 residency, board certifications (BPS), or equivalent training
