Responsibilities
Deliver high-quality primary care services to adult patients in an outpatient setting
Diagnose, treat, and manage acute and chronic medical conditions
Provide preventive care including physical exams, health screenings, immunizations, and wellness counseling
Develop and manage individualized treatment plans using evidence-based medical practices
Coordinate care with specialists, behavioral health providers, nursing staff, and care managers
Document all patient encounters accurately and timely in the electronic medical record (EMR)
Support population health initiatives and quality improvement programs
Educate patients on disease prevention, lifestyle modification, and medication management
Participate in interdisciplinary case conferences and clinical team meetings
Maintain compliance with all clinical, regulatory, and organizational standards
Provide continuity of care through appropriate follow-up and care coordination
Qualification
Required
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
Completion of an accredited Internal Medicine residency program
Active and unrestricted medical license in the applicable state
Board Certified or Board Eligible in Internal Medicine
Strong diagnostic, clinical decision-making, and communication skills
Commitment to working with underserved, diverse, or medically complex populations
Preferred
Prior experience in community health, FQHC, or public health settings preferred but not required
Bilingual skills a plus
