Responsibilities
Provide comprehensive onsite primary care to seniors in assisted living and senior living communities
Develop and execute personalized, patient-centered care plans that address the whole person
Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves
Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes
Qualification
Required
NP or PA certification
Genuine passion for geriatric medicine
Adaptable and resilient startup mentality
Comfortable providing onsite care across multiple senior communities
Committed to serving complex care patients with empathy and clinical excellence
Values multidisciplinary collaboration and diverse clinical perspectives
Strong interpersonal skills for engaging patients, families, care teams, and community partners
Ready to leverage digital tools including EMR systems, remote monitoring, and care coordination platforms
Open to scheduling variations and local travel between partner communities
