Sickle Cell Disease Program Director
Southern Jersey Family Medical Centers, Inc. is looking for a talented Sickle Cell Disease Program Director with the highest level of professionalism to oversee and coordinate comprehensive patient care. The role involves managing multidisciplinary teams, developing strategic plans, and ensuring high-quality, equitable services for SCD patients across the lifespan.

Responsibilities
Develops vision, strategy, and operational plans for the SCD program, aligning with grant and organizational goals
Manages care across all settings (inpatient, outpatient), acts as a liaison, and ensures seamless, whole-person care
Leads SCD Pilot Program team, fosters collaboration (hematology, pain, social work), builds partnerships with community groups, government, and donors
Designs, implements, and oversees community engagement strategies. Works collaboratively with marketing to create educational materials
Tracks program metrics, analyzes SCD data, prepares reports for leadership and state grant requirements, and ensures regulatory compliance
Excellent verbal and written communications skills
Other duties as assigned
Qualification
Required
Bachelor's degree in social work, public health, or related field required
Background in hematology, chronic disease management, social work (MSW), or public health
Strong project management, leadership, and coordination skills
Excellent written/verbal skills for diverse audiences (patients, clinicians, policymakers)
Ability to interpret and present data to for quarterly reporting and project success
Dedication to improving lives of people with SCD
Valid NJ Driver's License and reliable transportation
