Employment & Education Specialist - QMHP
Integral Care is the largest provider of behavioral health services in Travis County, dedicated to improving the lives of individuals with mental illness and substance use disorders. The Employment & Education Specialist - QMHP is responsible for providing recovery-focused, person-centered care, performing case management, and administrative duties while ensuring effective coordination of services and resources for clients.

Responsibilities
Ensure the provision of recovery-focused, person-centered care with a trauma-informed perspective
Perform case management, assessment, and/or service coordination work in accordance with the program's guidelines
Provide continuity of care services, linking consumers and families to all needed resources
Coordinate, monitor, and update services and consumer goals
Provide necessary skills training, assist in crisis prevention, and/or provide community outreach
Coordinate crisis intervention tools to address clients' needs, utilizing assessment and multidisciplinary coordination
Collaborate with consumers, first responders, and community stakeholders to address immediate mental health crises and plan for future crisis relapse prevention
Follow identified strategies for the achievement of unit performance measures and ensure the highest quality of care while effectively utilizing resources
May carry an individual caseload and may require on-call responsibilities
Qualification
Required
Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field
Current and valid driver's license and a clean driving record
