Responsibilities
Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a positive, informative, and patient-centered experience
Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive quality outcomes and overall patient experience
Document patient interactions, assessments, and intake in the EHR
Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and timely follow-up on patient needs
Assist with patient outreach and escalation to clinics when applicable and delegated from the nursing staff
Qualification
Required
Active MA license/certification and/or proof of completion of a nationally recognized program
Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must
Quiet, HIPAA-Safe space to do your work
Fast, reliable internet connection. A hardwired internet connection is required for this position
Benefits
Sign-On Bonus: This role is eligible for a one-time $2,500 sign-on bonus. The bonus will be paid after you complete 120 days of continuous employment and is contingent upon your active employment status and satisfactory performance at the time of payment.
Bonus potential: Earn up to $1,000 per month
