Senior CMHACS/Crisis Team Practitioner - Mental Health

They are seeking a Senior CMHACS/Crisis Team Practitioner to provide clinical leadership and support for individuals with complex mental health needs, ensuring effective service delivery and collaboration with other agencies.

Responsibilities

Provide a service for individuals with complex mental health, physical and social care needs who present in crisis and who may require intensive home treatment/care intervention, as an alternative to hospital admission
Be closely involved in discharge planning and immediate follow up from acute in service user care
Responsible for the clinical leadership of more junior members of the team, prioritising workload and utilising resources appropriately to ensure all service user needs are met
Accountable to the Community Mental Health Acute Care Service Leader for the management of their workload and the delegated functions to other team members
Practice within and ensure that the practice of junior team members is within legal and ethical frameworks according to appropriate codes of professional conduct, mental health legislation and contractual obligations
Assist the Team Leader in the rapid deployment of crisis resources in response to service user need
Interface between the Community Mental Health Team and In Service user services, and other mental Health Network services and agencies

Required

Experience in providing mental health care services to individuals with complex mental health, physical and social care needs
Ability to work within legal and ethical frameworks according to appropriate codes of professional conduct, mental health legislation and contractual obligations
Experience in clinical leadership and supervision of junior team members
Ability to prioritize workload and utilize resources appropriately to ensure all service user needs are met
Experience in discharge planning and follow-up care for individuals transitioning from acute services
Ability to interface effectively between Community Mental Health Teams and other mental health services and agencies
Experience in rapid response crisis intervention and deployment of crisis resources
