Responsibilities
Assess member needs by phone, home visits, or virtual outreach to determine care and service requirements
Create and implement personalized care plans for members with complex behavioral health or pediatric conditions
Identify and coordinate resources, specialists, and community support to meet medical and social needs
Monitor care plans and adjust as needed for changes in condition, treatment, or member progress
Collaborate with families, caregivers, healthcare providers, and community agencies to ensure continuity of care
Provide education to members and families about treatment options, healthcare instructions, and available benefits
Document assessments, care plans, and progress notes to maintain compliance with state, federal, and payer regulations
Advocate for members to ensure timely access to services and address barriers to care
Report outcomes and improvement opportunities to leadership for quality enhancement
Comply with organizational policies, procedures, and professional standards
Qualification
Required
Master's degree in Behavioral Health, Social Work, or Nursing
2 to 4 years of pediatric or related clinical experience
Active license as an LCSW, LMHC, LPC, or RN (as required by state)
For Florida CMS programs: Master's degree in social work or related field and at least one year of pediatric care experience
May require up to 80% local travel
Preferred
Behavioral health care management experience preferred
Experience in home visits, community health, or behavioral health
Bilingual (Spanish) is a plus
