Responsibilities
Reviews non-compliance lists and conducts outreach to ensure timely disease specific and wellness visits and tests with submission to the respective payer
Completes quality, safety and other ad hoc reporting on a monthly basis
Prepares payer continuity of care forms for provider completion
Reviews monthly payer reports related to medication adherence, open care gaps, and HCC/Raf opportunities and communicates outliers to respective practice to complete
Participates in practice safety walkthroughs and proactively helps to identify potential areas of risk
Help to facilitate new hire and annual competency assessments
Qualification
Required
Must have a high school diploma/GED
2 years working in an outpatient medical practice
Preferred
Associates degree
Experience in primary care setting
