Responsibilities
Perform comprehensive assessments of hospice patients
Collaborate with primary care providers to develop and implement individualized, comprehensive care plans
Continuously reassess appropriate and cost-efficient levels of care
Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
Arrange non-medical support services (such as housing and transportation) to ensure compliance with treatment plans
Engage specialty resources as needed to achieve optimal patient outcomes
Maintain detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient occurrences when possible and intervene promptly when necessary
Act as a patient advocate while protecting patient privacy and confidentiality
Qualification
Required
Associate's or Bachelor's degree in Nursing (or related field)
Current, full, active, and unrestricted Minnesota Registered Nurse (RN) license
Hospice experience required
Strong ability to analyze complex medical information and make sound clinical decisions
Preferred
Prior case management experience
Experience coordinating care across multidisciplinary teams
