Responsibilities
Provide comprehensive, hands-on assistance with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and transfers, always prioritizing resident comfort and autonomy
Diligently monitor and track changes in resident physical condition, behavior, and vital signs; serve as a vital link by promptly reporting observations to the supervising nurse
Support residents with medication reminders or administration as permitted under CBRF regulations and facility protocols
Foster social interaction by encouraging and assisting residents with scheduled activities, therapy programs, and community events
Maintain a safe, hygienic, and organized living space for residents, including room tidying and linen management
Ensure all care services and resident updates are recorded accurately and promptly in the facility’s electronic documentation system
Strictly adhere to all local, state, and federal healthcare guidelines to ensure the unit operates at the highest level of regulatory excellence
Qualification
Required
Must be CBRF Certified in the state of Wisconsin
High School Diploma or equivalent
Proven ability to work independently with minimal supervision while remaining a reliable, proactive team player
Basic computer skills required for electronic charting and communication
Preferred
Previous long-term care or assisted living experience is strongly preferred; however, we welcome heart-led individuals ready to grow with us
