Coordinate the admission process from assessment to move-in, ensuring a seamless transition for new residents and their families
Conduct pre-admission interviews and assessments to gather essential information and develop individualized care plans
Collaborate with interdisciplinary team members to ensure a comprehensive understanding of resident needs and develop effective care strategies
Coordinate necessary documentation, paperwork, and administrative tasks to facilitate the admission process
Develop and maintain relationships with referral sources, including hospitals, rehabilitation centers, and other healthcare organizations
Provide exceptional customer service to residents, families, and referral sources, ensuring a high level of satisfaction and loyalty
Maintain confidentiality and adhere to HIPAA regulations at all times
