Case Manager - C.O.Y.A.
Amity Foundation is an internationally acclaimed Teaching and Therapeutic Community seeking compassionate individuals for the role of Case Manager. This position involves working with youth ages 18–24 facing various barriers and requires conducting assessments, developing case plans, and coordinating with community organizations to support their transition into employment or education.

Responsibilities
Conduct comprehensive intake assessments for assigned students, identifying strengths, needs, and barriers
Develop, review, and regularly update Individual Development Plans (IDPs) tailored to each student’s goals emphasizing education in substance use disorder, employment, and supportive services
Provide personalized mentoring, including weekly one-on-one and group sessions focused on soft skills, motivational interviewing, problem-solving, crisis management, and trauma-informed care
Coordinate closely with Cuesta College and East Los Angeles College and other community-based organizations
Support students in acquiring employment skills, preparing resumes, practicing interview techniques, and securing job placements
Facilitate student’s enrollment in educational programs and assist with financial aid applications when applicable
Document all progress, services provided, and employment outcomes thoroughly in the student’s files and the Apricot data management system
Regularly meet with students to align mentoring activities with students
Monitor student progress and regularly coordinate with Community-Based Organizations
Make appropriate referrals to external agencies for additional support services as necessary
Maintain comprehensive case records and accurate statistical data for students ensuring documentation meets all compliance requirements
Regularly review student’s outcome data with the Associate Director of Employment Services to make timely adjustments that enhance program effectiveness
Actively engage and communicate with students' families or caregivers to support a holistic approach to participant development and employment sustainability
Participate actively in community-building events, collaborative stakeholder meetings, workshops, and relevant training
Maintain effective communication and collaborative relationships with stakeholders, and community partners
Serve as a community advocate as requested by program leadership
Qualification
Required
Bachelor's degree in social sciences, youth development, rehabilitation counseling, workforce development, or a related field; OR High school diploma or equivalent with a minimum of two years' experience working with justice-involved youth, individuals with disabilities, or related populations
Valid California driver's license and eligibility for Amity vehicle insurance
Understanding of causes, symptoms, and effects of long-term incarceration; appreciation for the behavioral, psychological, physical, and social effects of incarceration
Recognition of the importance of individual differences created by personalities, cultures, lifestyles, social networks, and other factors that influence behavior
Operational knowledge of MS Word, Excel, and Access
Ability to prioritize competing tasks with multiple deadlines
Excellent interpersonal and communications skills and the ability to work effectively with a wide range of constituencies in the community and within the organization
Demonstrate ethical behavior to maintain professional standards and safeguard the participant
Participate in community-building activities as appropriate to the position
Protect program participants' health, safety, welfare, and confidentiality of records and participant information
Ability to change direction as priorities shift
Ability to thrive in an environment that requires one to adapt to changing circumstances
Ability to professionally work under pressure with difficult or temperamental students & faculty
Ability to handle mental and emotional stress related to the position
Preferred
Experience providing case management services within juvenile justice or rehabilitation settings
Certification in motivational interviewing or trauma-informed care practices
