Responsibilities
Provide individual/family therapy and document progress toward goals in EHR
Monitor consumer's progress and effectiveness of strategies and interventions outlined in the person-centered plan
Lead any and all revisions of the Crisis Plan following a crisis
Meet with IIH staff to discuss consumer's progress
Provide initial and ongoing psychoeducation to team members, consumer, family, and stakeholders
Actively participate in weekly IIH meetings and provide therapeutic strategies and input to team members
Actively participate in weekly supervision with Program Manager and Clinical Supervisor
Actively participate in and provide clinical expertise during CFTs
Provide clinical supervision of all members of the team for the provision of this service
Re-assess each consumer and complete a CCA every six months. CCA shall include recommendations for continued stay/transition and or discharge
Qualification
Required
Must be fully licensed (or provisionally licensed) by the appropriate North Carolina licensure board as a Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Professional Counselor, Psychiatrist, Licensed Psychologist, or a Licensed Psychological Associate
Preferred
LCSW, LCMHC, LMFT, LPC preferred
